understand that participating in activities at Break Point
NY, including but not limited to rage room smashing, axe throwing, and use of protective gear,
involves risks of serious injury, property damage, or death. I voluntarily assume all risks, both
known and unknown, even those arising from negligence.
2. RELEASE OF LIABILITY In consideration for being allowed to participate, I hereby release and
hold harmless Break Point NY LLC, its owners, employees, agents, contractors, and
representatives from any and all liability, claims, demands, or causes of action arising out of my
participation, including injury, death, or property damage.
3. SAFETY RULES & EQUIPMENT- I agree to follow all posted and verbal safety instructions from
staff. - I will wear required protective gear (helmet, gloves, goggles, closed-toe shoes, etc.). - |
will not throw, swing, or use equipment recklessly. - I understand that intoxication is prohibited
and may result in removal with no refund.
4. AGE & HEALTH CERTIFICATION- I certify that I am 18 years or older (or have a parent/legal
guardian present to sign). - I have no medical, physical, or mental conditions that would make
participation unsafe.
